Individuals 12 years or older may receive the Pfizer vaccine.
Individuals 18 years or older may receive the Moderna or Johnson & Johnson vaccine.

HOW DO I GET THE VACCINE?
Brown County Health Department will be disributing the vaccine at our office. Please contact our Nursing Department to schedule an appointment. If you wish to schedule an appointment at Kroger or HealthSource please contact their facilities, their contact information is listed under the Where can I get the vaccine? section.
WHAT WILL I NEED TO BRING WITH ME?
When you come to our office you will need the following items:
- Completed COVID-19 Vaccine Registration Form (For 3rd Dose please use this Registration Form)
- Insurance card
- Proof of ID (to verify age)
WHAT HAPPENS AFTER I GET THE VACCINE?
You will be asked to wait for 15 minutes after you have received your vaccine. Our staff must monitor each patient for any allergic reactions. You will receive a vaccine record card showing that you have received the COVID-19 vaccine (do not lose this record) and you will be told the date and time of when to return for your second dose.
